As your employer we are required by the ACA to provide you with the attached "Notice of Health Care Marketplace" which provides you with required employer and plan information. You will need to refer to this notice if you choose to enroll in individual coverage through the "Marketplace".

2014 W-2 Reprints
2014 W-2's have been mailed out and may take up to 7 business days to receive. If you have not received your 2014 W-2 by February 9th, you may contact our accounting department at (567) 661-0609. Please leave a detailed message and your call will be returned within 2 business days.
To request a W-2 reprint, you will need to mail us the following:
  • Your name
  • The last 4 digits of your Social Security number
  • Your phone number
  • Your current mailing address
  • The year of the W-2 that you need reprinted
  • Processing fee payment in the form of a check or money order, made payable to Job1USA in the amount of $5.00

If the request is received after 3/31/2015, the processing fee will be $10.00.
